*STUDENTS MUST HAVE A PASS TO COME TO THE NURSE'S OFFICE*
Please report all school injuries to the nurse. All gym excuses and hall passes will be issued by the nurse with an accompanying note from the student's physician.
EMERGENCY CONTACT CARDS
An updated contact card is required each year. Please update any changes in phone numbers, guardians or employment when they occur. Central Registration must also be contacted so that your information is updated in the school-wide computer system.
Immunizations
All students must have the following immunization to attend school:
Diphtheria toxoid-containing vaccine, Tetanus toxoid-containing vaccine and Pertussis vaccine (Dtap): 3 doses (only need a minimum of 3 to get into school).
Polio - 3 doses
MMR-2 or 2 doses measles and 1 dose MMR
Hepatitis B - 3 doses
Varicella - 2 doses
TDAP Booster at age 11 or 6th grade
Administration of Medications in School
In order to administer medication to your child during school the attached form must be completed and signed by your physician. A parent must also sign the form. See link below.
Physicals
9th and 11th graders are required to have state mandated physicals. These physicals must be performed by November of each year. The school physician can perform 10th grade physicals if requested by parent or guardian. Any student who does not have the mandated physical will be excluded from school. See link below.
Sports Physicals
Any student wishing to participate in sports must have a YEARLY sports physical and PERMISSION SLIP for each sport. Sports physical forms and permission slips can be obtained in the Health Office. Permission slips must be completed and signed by a parent or guardian. The school physician performs sports physicals three times a year prior to each sports season. See link below.
